What Is Adult ADHD?

Attention-Deficit/Hyperactivity Disorder (ADHD) is one of the most common neurodevelopmental disorders in children and adolescents. However, it is not exclusively a childhood condition. Approximately 50% of individuals diagnosed in childhood continue to experience symptoms into adulthood, resulting in an estimated prevalence of around 3.5% among adults aged 18 to 45 years.

Adult ADHD frequently coexists with other mental health conditions such as anxiety disorders, depression, or substance use disorders, making diagnosis more complex.

Although some recent studies have suggested the possibility of adult-onset ADHD, this remains controversial. Many researchers believe these cases may represent a different clinical syndrome rather than the same neurodevelopmental disorder.

Causes of Adult ADHD

Although the exact cause remains incompletely understood, current evidence suggests that ADHD is associated with dysfunction in the brain networks connecting the frontal, parietal, and subcortical regions, areas responsible for attention, executive functioning, motivation, and behavioral control.

  • Symptoms occurring across multiple settings (work, home, education, or social environments)
  • Significant impairment in daily functioning
  • Symptoms that cannot be better explained by another psychiatric or medical condition

Diagnostic Tools

Several standardized instruments can support the clinical evaluation, including:

  • DIVA 2.0 (Diagnostic Interview for ADHD in Adults), a structured diagnostic interview.
  • CAARS (Conners’ Adult ADHD Rating Scales).
  • ASRS (Adult ADHD Self-Report Scale).

These tools complement—but do not replace—a full psychiatric assessment.

Obtaining collateral information from family members or others who know the patient well is often extremely valuable.

Differential Diagnosis

Because ADHD frequently coexists with other psychiatric disorders, clinicians must carefully evaluate for:

  • Substance use disorders
  • Bipolar disorder (mania or cyclothymia)
  • Personality disorders

Identifying coexisting conditions is essential for accurate diagnosis and appropriate treatment planning.

Treatment of Adult ADHD

The most effective treatment generally combines medication with psychological therapy.

Psychological Therapy

The strongest evidence supports Cognitive Behavioral Therapy (CBT) specifically adapted for adult ADHD, particularly interventions targeting executive functioning, organization, emotional regulation, and coping strategies.

Medications

First-line treatment: Stimulants

Stimulant medications are considered the first-line pharmacological treatment and include:

  • Dextroamphetamine
  • Lisdexamfetamine
  • Methylphenidate
  • Dexmethylphenidate

Before prescribing stimulants, clinicians should perform a thorough medical evaluation to identify contraindications, potential drug interactions, and cardiovascular risk factors.

Patients should also be monitored for possible side effects, including:

  • Insomnia
  • Elevated blood pressure
  • Weight loss

Non-stimulant alternatives

When stimulants are ineffective or contraindicated, alternative medications include:

  1. Selective norepinephrine reuptake inhibitors
    • Atomoxetine
    • Viloxazine
  2. Dopamine and norepinephrine reuptake inhibitor
    • Bupropion
  3. Tricyclic antidepressants
    • Nortriptyline
